| New personalised care network launched by the DoH | Posted on 02/02/2010 in Medical Government/ NHS related news The Department of Health (DoH) has announced the launch of a new personalised care service, which is designed to boost the level of support patients receive in local communities.
Health minister Mike O'Brien and care services minister Phil Hope made the announcement this week, adding that the self-assessment tool will give health and social care services the chance to join the new, extended integrated care community.
The Integrated Care Pilots scheme, which was launched in August last year, will provide the building blocks for the expanded service.
Mr O'Brien stated that the aim is to get social and health care facilities working "seamlessly" together.
"We know that, for example, patients overcoming substance misuse can frequently have housing problems or social care needs as well as health ones," he explained.
This is not the only welfare scheme announced by the DoH within the last week, as it recently unveiled a new campaign to discourage people from consuming excessive amounts of alcohol. Other news stories from 02/02/2010
